House Calls for Seniors is a program designed to provide team-based primary care in the home for those age 65 and over who live in Marion County and are unable to leave their homes without extraordinary assistance due to physical or psychosocial limitations.
House Calls for Seniors accepts patients through physician, agency or family referrals. Once a patient is referred, he or she is screened. The patient then accepts the House Calls team - a geriatrician, nurse practitioner and social worker - as his or her primary care providers and accept Wishard Memorial Hospital as his or her primary hospital.
