Extended Care
The Extended Care program provides rehabilitation services or long-term care in a nursing home to patients through a team of doctors and nurse practitioners from IU Medical Group. This allows continuity with a patient’s IU Medical Group doctors and the Wishard Senior Care program.
Medical care can be provided in many nursing facilities.
